It doesn't look like you followed the installation instructions properly.  The url (/createuseraccount.aspx) is not a url that is in the portal, and the port is 4444 which both sound like that is your CRM service, yet the error that is thrown is showing that you are using an app pool for CRM that looks like the customer portal app pool.  The portal and CRM are completely separate sites and you should make NO modifications to your IIS site hosting your CRM when you are setting up the customer portal.  The portal runs as a standalone website, and in most environments, it runs on another server which is typically dedicated to web hosting.  If you have changed your CRM IIS website, I would recommend that you undo the changes you did, then refer back to the installation documentation for the customer portal and read it carefully.
