Case notes are duplicating on Portal

I'm using the eService portal for the first time.

I'm logging in as a customer on the portal and create new case. ->The Case is created in CRM.

Then I go to the portal and add a Note to the case ->the note is created in CRM

I create a note in CRM without the Web prefix, refresh the portal -> on the portal the case is updated by a new note that is just a replicate of the first note the customer has created.

I create a note in CRM with the prefix and refresh the portal->on the portal a note is added as a replicate to the first note again

this keeps hapening on and on, portal seems to replicate the last customer's note everytime.

Has anyone encountered this issue before?
